In accordance with IRS regulations, CSM Group will not automatically provide Form 1095-C to employees. However, employees may request a copy of their 1095-C if desired.
To request a copy of your Form 1095-C, please contact the People Operations department by emailing benefits@csmgroup.com or calling (877) 386-8214. You may also submit your request in writing to the following address:

Requests will be processed within 30 days of receipt. This notice will remain available on our website until the end of the calendar year following the year to which the Form 1095-C relates.
